- Youth Plan: Up to 2 exams, Routine Cleanings, and recommended X-rays
- Adult Plan: Up to 2 exams, Routine Cleanings, and recommended X-rays
- Active Periodontal Plan: Up to 2 exams, Scaling and Root Planing, 3 Periodontal Maintenance, recommended X-rays.
- Periodontal Maintenance Plan: Up to 2 exams, 4 Periodontal maintenance cleanings, and recommended X-rays.
- 1 – emergency visit per year
- 1 – oral cancer screening per year
- 10% Discount on all other dental treatment.
